We are seeking a compassionate, motivated, and dependable Registered Nurse (RN) to join our team full-time. Responsibilities
- Complete Home Health Start of Care (SOC) admissions in patients' homes.
- Perform comprehensive nursing assessments and develop individualized plans of care.
- Accurately complete OASIS documentation and all required clinical records.
- Provide clinical support in the office when not conducting admissions.
- Ensure compliance with Medicare regulations, agency policies, and home health standards.
- Collaborate with the interdisciplinary team to provide high-quality patient care.
- Deliver compassionate, patient-centered care while promoting safety and independence.
Qualifications
- Active Massachusetts Registered Nurse (RN) license.
- Valid driver's license and reliable transportation.
- Strong clinical assessment, communication, and documentation skills.
- Ability to work independently and effectively manage time in a fast-paced environment.
- Excellent organizational and problem-solving abilities.
Preferred Qualifications
- Home health experience preferred.
- OASIS documentation experience preferred.
- Spanish-speaking is a plus but not required.
