As the Director of Social Services, you will use your assessment and evaluation skills to assist residents and their families with the psychosocial and emotional issues related to illness or aging.

Essential Responsibilities
- Assist in planning, organizing, implementing, evaluating, and directing the overall operation of the Social work department
- Ensure services are provided in accordance with current Federal, State and local standards/guidelines and regulations
- Coordinate appropriate discharge planning options
Qualifications
- Qualified candidates must possess a LSW or LCSW and long term care experience.
- experience in SNF setting is required
- Ability to establish trusting relationships with patients and their families to plan for the next chapter in their care.
- Ability to assess needs from admission to discharge, develop treatment plans and document interventions/information in medical records.
