Position Overview
We are seeking a compassionate, detail-oriented Case Manager to join our luxury primary mental health PHP/IOP treatment center. This role is essential in supporting clients throughout their treatment journey by coordinating care, facilitating discharge planning, and ensuring continuity of services. The ideal candidate brings at least two years of case management experience, preferably within a primary mental health treatment setting, and excels in a client-centered, high-touch clinical environment.
Schedule: Monday - Thursday 10:00am - 6:00pm, Friday - 9:00am - 5:00pm
Key Responsibilities
- Serve as a primary point of contact for clients regarding care coordination and treatment planning
- Collaborate with the multidisciplinary clinical team to support individualized treatment plans
- Coordinate referrals and aftercare planning, including outpatient providers, psychiatry, therapists, and community resources
- Assist clients with scheduling appointments, accessing resources, and navigating levels of care
- Conduct ongoing check-ins to assess client progress, needs, and readiness for transition or discharge
- Maintain accurate, timely, and compliant clinical documentation
- Communicate professionally with families, external providers, and referral sources as appropriate
- Ensure a high standard of client experience aligned with a luxury treatment environment
- Support utilization review and insurance-related coordination as needed
Qualifications
- Minimum 2 years of experience as a Case Manager in behavioral health or mental health treatment
- Experience working with a primary mental health population strongly preferred
- Bachelor’s degree in Social Work, Psychology, Counseling, or a related field required (Master’s degree preferred)
- Strong understanding of PHP/IOP levels of care and treatment continuity
- Excellent communication, organization, and time-management skills
- Ability to work collaboratively within a multidisciplinary clinical team
- High level of professionalism, discretion, and empathy
Preferred Experience
- Prior experience in a PHP/IOP or luxury treatment setting
- Familiarity with EMR systems and clinical documentation standards
- Experience with discharge planning and community resource coordination

Pay: $50,000.00 - $60,000.00 per year
