Overview
We are seeking a dedicated and compassionate Registered Nurse (RN) to join our Adult Day Health Center team. The ideal candidate will possess a strong background in patient care, with experience in various medical settings. This role involves providing high-quality nursing care to adults, including those with developmental disabilities and chronic health conditions. The RN will work collaboratively with a multidisciplinary team to ensure the best possible outcomes for our clients.
Responsibilities
- Conduct comprehensive patient assessments and develop individualized care plans.
- Administer medications and treatments as prescribed, ensuring adherence to safety protocols.
- Monitor vital signs and respond to changes in patients' conditions promptly.
- Provide education and support to patients and their families regarding health management.
- Document all patient interactions, assessments, and care provided accurately in electronic medical records (EMR).
- Collaborate with physicians, therapists, and other healthcare professionals to coordinate care.
- Assist with procedures such as IV infusion, catheterization, and wound care.
- Participate in quality improvement initiatives and adhere to NCQA standards.
- Ensure compliance with HIPAA regulations and maintain confidentiality of patient information.
Qualifications
- Current RN license in the state of practice.
- Experience in acute care, long-term care, or assisted living settings is preferred.
- Proficiency in EMR systems such as Epic or Cerner is highly desirable.
- Familiarity with ICD-9 and ICD-10 coding for accurate medical documentation.
- Knowledge of critical care practices, including ventilator management and acute pain management.
- Experience working with individuals with developmental disabilities or in memory care settings is a plus.
- Strong communication skills and the ability to work effectively within a team environment.
- Basic Life Support (BLS) certification required; Advanced Cardiac Life Support (ACLS) certification preferred.
