Responsibilities:
- Under the direction and supervision of the Director of Advocacy & Family Support per the AFC Regulations the licensed practical nurses may:
- Complete and coordinate all applicable clinical assessments and clinical evaluations;
- Develop and review on an ongoing basis each member’s AFC plan of care;
- Provide oversight to AFC caregivers in conjunction with the care manager;
- Review the PCP Summary Form;
- Monitor the health status of all members to ensure that all needed AFC is properly delivered;
- Report changes in the health status of any member to the member’s PCP;
Conduct on-site visits with each member at the qualified setting in accordance with 130 CMR 408.415(B)(6);

- Licensed Practical Nurse with two years experience with ID/DD populations or the elderly. Current license in the State of Massachusetts.Valid driver’s license and insured vehicle.
