Key Responsibilities

Conduct comprehensive nursing assessments addressing physical, emotional, psychosocial, and spiritual patient needs.
Collaborate with physicians and interdisciplinary care teams to create, implement, and evaluate individualized care plans.
Provide skilled nursing care including:
- Medication administration
- IV therapy
- Wound care
- Symptom and pain management
- Patient and caregiver education
Monitor patient progress and modify care plans based on ongoing evaluations and clinical findings.
Deliver compassionate emotional support and health education to patients and their families.
Supervise LPNs/LVNs and home health aides while coordinating daily care activities and documentation.
Respond promptly to urgent patient needs and participate in admissions and on-call responsibilities as required.
Maintain accurate, timely, and compliant clinical documentation in accordance with healthcare regulations and company standards.
Ensure delivery of high-quality, safe, and compassionate patient care.

Qualifications

Required Education

Associate’s Degree or Graduate Diploma in Nursing from an accredited nursing program.

Required Experience

Minimum of one (1) year experience as a Registered Nurse.

Required Licensure & Certifications

Active Registered Nurse (RN) license in the state of Minnesota.
Current CPR certification required.
Valid driver’s license with reliable insured transportation.
