Licensed Practical Nurse (LPN) – Home Care

Are you a compassionate LPN looking to make a real difference in patients' lives — right in the comfort of their own homes? Havenovacare is hiring Licensed Practical Nurses for Full-Time, Part-Time, and Per Diem/PRN positions across Massachusetts. You will provide hands-on nursing care to our patients under the supervision of a Registered Nurse or physician, with the flexibility to balance your schedule while delivering exceptional care. What You'll Do
• Provide direct, in-home patient care including medication administration, wound care, catheter care, and prescribed treatments
• Monitor and document patient vital signs, health status, and any changes in condition
• Implement individualized care plans in collaboration with RNs, physicians, and the broader care team
• Educate patients and family members on medications, procedures, and disease management
• Maintain accurate, timely clinical documentation in compliance with agency and regulatory standards
• Communicate proactively with supervisors and care teams regarding patient progress
• Follow all infection control protocols, safety standards, and HIPAA regulations
• Attend care conferences and team meetings as needed
What We're Looking For
• Current, valid LPN license in the State of Massachusetts (in good standing)
• Minimum 1 year of clinical nursing experience; home care experience preferred
• Current CPR/BLS certification
• Valid driver's license and reliable personal vehicle
• Ability to pass a criminal background check and pre-employment health screening
• Availability to work weekends (required)
• Bilingual (English + any additional language) strongly preferred
• Proficiency with electronic health record (EHR) systems is a plus
